Which file, the zip file initially uploaded is now the one I should take?

Yes, that's the right one.
I always delete the old one before uploading a new version.

ok. it does all work great but there is now a login issue it would seem....

Thanks to Thomas, who has found the issue.
I uploaded new versions of the module and the update script (which is only needed if you update a really outdated version, see this thread :-)).
You should all update the module, as users can't login after their messages are rated. I wrote an announcement at my forums and included a link to it in the error message, that tells the user, that their login is wrong. If the user gets himself a new password, the login will work again.

But that doesn't affect the indicator image.
I uploaded a new version where you can do this with a template change. Add the following variable to your read.tpl:
